How to ask for a letter of recommendation
Send me an email at least two weeks in advance of when you need the letter. If it is less, acknowledge that it is less and be ready to provide as many of the materials as you can right away.
Tell me what the program/job/opportunity for which you're applying is, why you're applying for it, and when the deadline for the letter is. If there is no specific deadline (e.g., applications are rolling), tell me when you're planning to submit your application.
Provide me with a copy of your resume and any other application materials you have (even if they're in draft form).
You should also give me any other relevant information (e.g., will I receive a link to submit the reference? Does it need to be on letterhead? Is there anything specific the hiring committee, etc., wants letter-writers to address?
I am often happy to give feedback on your application materials; please let me know if you'd like this feedback and consider making an appointment to talk about it.
There might be times that a letter of recommendation from me would not be the most appropriate. This does not mean that I don't like you as a person! Feel free to ask me about it, and we can have that conversation.

I'm writing because I'm applying to [an amazing opportunity], and I would be grateful if you'd be willing to [provide a reference/letter of recommendation/whatever] for me.
I am applying to [the opportunity] because [a reason or two about why I am interested and qualified]. A copy of my application materials is attached to give additional context for my application.
The reference is due on [date]. [A link will be sent to you once I submit my application/you can submit the letter on official letterhead to this email address/other instructions.]
I understand if you don't have capacity to write for me, but please do let me know as soon as possible. Please let me know if you have any questions or need additional information.
